1 - Aircraft Rates - Where can I find them?      (back to top)

2 - Form 5 - What's involved?     Lots of little pieces!      (back to top)
  2-1   You need to take the online CAPR 60-1 written test each year.
  The info needed to answer ALL questions can be found in CAPR 60-1
There are 50 questions, no time limit, open book and reqiures a minimum of 80% correct answers.

- Scroll down and click on "R60-1" in the left (1st) column.
    NOTE: As if this isn't hard enough, there are 4 rows that have R60-1.
    Click on the 1st R60-1 (clicking on the next 3 R60-1 won't do anything)
- If you are lazy like me, this link (CAPR 60-1) works as of 10/20/2011 but might not be the latest CAPR 60-1.

To take the exam, login to e-Services and then go to:

From the dropdown box “*Select Quiz”
- SELECT: CAPR 60-1 CAPF 5 Annual Examination – Powered

At the successful completion of the test, print a copy of the completion certificate for your own records.
If needed, you can get a copy at a later date but having your own backup is a good idea.

  2-2   Each year, you must also take a CAP Airplane Questionaire FOR EACH AIRCRAFT you intend to fly.
The Airplane Questionnaire can be found at: http://members.gocivilairpatrol.com/forms_publications__regulations/forms.cfm
- On the row just below "**F5" that says "Airplane Questionnaire"
    in the 5th column, click on the "W" icon to get the WORD document on which you can enter data.
- If you are lazy like me, this link (http://members.gocivilairpatrol.com/media/cms/Questionnaire_Airplane_1D2124EF34D91.doc)
    works as of 10/21/2011 (revised August 2009) but might not be the latest Airplane Questionnaire.

You will need the POH to correctly answer these questions.
- to the best of my knowledge, you will need to look at the POH in the plane since it is not yet online.

  2-3   You need to be FAA current.
  • Current Flight Review
  • Current Medical
  • 3 take offs and landings in the previous 90 days (to carry passengers).

  2-4   The list of knowledge and skills you will need to demonstrate are listed on CAPF 5 (Form 5)

BEFORE you go to the actual flight test, please fill out 2 sections:

  2-5   When you are ready to take the Form 5 flight test:
  • Setup a date with the Check Pilot of your choice
    - To see a list of current Check Pilots:
        - go to the NY Wing website: http://nywcap.org/index.html
        - if you do NOT have a NYW E-Business login, select the "To register for E-Business" link near the bottom of the page to create an account.
        - when you DO have a NYW E-Business login, select the "E-Business Login(already registered)" link near the bottom of the page to login.
        - after logging into the New York Wing E-Business website,
           SELECT "Pilot Reports" from the dropdown box and hit the "Submit" button.     - on the NYW Pilot Reports page,
           click on the "Check Pilot Authorization - NYW Check Pilots" link
           - NOTE: they are listed by Group, e.g., FLG, CNY, LIG, etc. so look for one in YOUR GROUP.
     
  • Add a SORTIE to WMIRS
    - Login to WMIRS at: https://missions.cap.af.mil/login.cfm
    - Login to WMIRS and enter a sortie:     https://missions.cap.af.mil/login.cfm
    • Select "Current Missions/Sorties" from left side Menu
    • After you find the Mission Number and click on the "Air" column link (the AIR number, NOT the mission number)
    • Click on "Add" near the bottom left to add your sortie to this mission.
    • Select the FRO you will be contacting and fill in the remaining appropriate data.
          When finished, hit "Add Sortie" at the bottom of the page.
    • Click on the "104" button on the right side of YOUR SORTIE to display your 104 form.
          Hit the "Printable PDF" button at the top of the page to get a hard copy to take with you on the mission.

  • If this is your first form 5, the check pilot will need to schedule the aircraft.
    After your first form 5, you can schedule the plane via WMIRS

  2-6   On the day of the Form 5 flight test ensure you have:
  • Your current CAP ID Card
  • Proof of your current Flight Review (Pilot logbook)
  • Proof of 3 take offs and landings in the previous 90 days (Pilot logbook)
  • Certificate of Completion for the CAPR 60-1 Annual Exam
  • Completed Aircraft Questionnaire (one for each aircraft you intend to fly)
  • Your Current Medical Certificate
  • Your Pilot License
  • Form CAPF 5 (With the TOP section and Certificates section already filled in)
  • A Flight Release
         - The Check Pilot may need to obtain the flight release if you aren't current)
Try to relax and fly like you do when the check pilot isn't there!

  2-7 After you have passed your Form 5 flight test you're STILL not done!
2-7-1   Let the FRO know that you are back safely.
2-7-2   From your check pilot, ensure that you have:
  • Your filled out and signed Form 5
  • A signed copy of each airplane questionaire.
2-7-3   Scan the Form 5 and Airplane Questionaire(s) to create separate PDFs.
2-7-4   Log in to eServices https://www.capnhq.gov/CAP.eServices.Web/Default.aspx
  • Click on My Operations Qualifications (on th left side)
  • Click on Airplane (under "Pilot - Checkride/Questionaires")
  • Near the top of the page, click on the View/Upload Documents link
            - You should now see a new popup screen which will allow you to upload your Form 5 and Airplane Questionaire PDF.
            - There are two tabs:   Emergency Services   and   Pilot   (You should be on the Pilot tab)
            - Select "Form 5" from the What would you like to upload? dropdown box.
            - Select the aircraft type, e.g., "C182/G1000" from the Aircraft Type dropdown box.
            - Hit the Browse... button and select the PDF copy on your computer.
            - Hit the Upload Pilot Files button
            - Repeat for the Questionaire(s)...
            - Click the X in the top right corner to close the popup screen.
  • In Questionaires Section
            - Enter the date the questionaire was graded.
            - AFTER entering the date, the Questionaire Aircraft Type dropdown box will populate...
            - Select the aircraft(s) for which you have a questionaire, e.g., C182/G1000
  • In the Annual/Abbreviated Form 5 Section
            - Select the Annual or Abbreviated checkbox.
            - Enter the Form 5 Date
            - Select the "Wing" checkbox.
            - AFTER you select the "Wing" checkbox, the Check Pilot dropdown box will populate
            - Select the name of your Check Pilot.
            - Select the Form 5 Aircraft Type, e.g., C182/G1000
              NOTE: The Form 5 Aircraft Type dropdown won't populate until AFTER you have entered a Form 5 Date.
  • In the Additional Endorsements Section
            - IF you did any other endorsements on this checkride, select the appropriate checkbox(es).
              NOTE: If you check any of these, ensure that they are also checked on your actual Form 5 sheet.
  • FINALLY, at the bottom (or top) of the page, HIT the Submit button.

4 - Where can I find the Weight & Balance Spreadsheets?      (back to top)

  4-2-2 Select "Pilot Reports" from the dropdown
  4-2-3 Click on the "NYW Aircraft weight and balance" link (near the bottom)

5 - What do I need to do for a mission? (back to top)
  5-1 Login to WMIRS and enter/add a sortie.     https://missions.cap.af.mil/login.cfm
  • Select "Current Missions/Sorties" from left side Menu
  • After you find the Mission Number and click on the "Air" column link (the AIR number, NOT the mission number)
  • Click on "Add" near the bottom left to add your sortie to this mission.
  • Select the FRO you will be contacting and fill in the remaining appropriate data.
        When finished, hit "Add Sortie" at the bottom of the page.
  • Click on the "104" button on the right side of YOUR SORTIE to display your 104 form.
        Hit the "Printable PDF" button at the top of the page to get a hard copy to take with you on the mission.
  5-2 Do a Weight and Balance. The Excel Spreadsheet is available at:

  • Ensure that you are on the correct spreadsheet tab for the aircraft you will be flying.
  • Get the weight of each passenger.
  • Determine how much fuel is in the aircraft.
  • Baggage weight?
  • NOTE: The repeater, if included weights 45 lbs.
  • Print the completed weight and balance.
  5-3 Do a RISK ASSESSMENT for this flight. Same as 5-2, the form is available at:
    http://ny412.nywg.cap.gov/Multimedia/Forms/tabid/186/ItemId/23/Default.aspx
  • Call Flight Service (1-800-WX-BRIEF) and get a briefing for this flight.
  • You will need current weather info.
  • You will need the mission number and sortie number (obtained in step 1 from WMIRS).
  5-4 Call/talk to a FRO (Flight Release Officer) to obtain your FLIGHT RELEASE

  • Just selecting a FRO in step 5-1 isn't enought.
        You need to talk to the FRO before you are officially released.
  • One might think that clicking on "e-Flight Release" in WMIRS would be the obvious path but it isn't...
  • You will need current weather info.
  • You will need the mission number and sortie number (obtained in step 1 from WMIRS).
  5-5 AFTER the flight - on your paper copy of the 104 form (from step 5-1), ensure you record:
  • TACH (start and end)
  • HOBBS (start and end)
  • FUEL used
  • Various mission times (wheels up, landing, etc.)
  5-6 AFTER the mission - again login to WMIRS and update your sortie data.     https://missions.cap.af.mil/login.cfm
5-6-1 If you have a fuel receipt, you will need to:
  • Create an electronic copy of the fuel receipt as a PDF document
  • There is a scanner in the squad room which you may use to scan your receipt and create the PDF document
  • Upload the PDF fuel receipt document to WMIRS, attached to your SORTIE
5-6-2 You will also need to fill out NYWF 108:
  • Date, Mission #, Tail Number, Hrs Flown, End Tach, Fuel Cost, Gallons
  • e-Mail the completed NYWF 108 Word document to   LtCol Bill Hughes at:   wnhughes@hughes.net
5-6-3 Can't find your sortie in WMIRS? - One last "tip"...
  • If you click on Current Missions/Sorties and then click on the AIR column to see your sortie
    and you don't see your sortie, click on the "Sort by Date" dropdown and select the date of your sortie.
    Hopefully you will see it now.
